Register for our COVID-19 Testing Program Today
Hello Creswell Middle Parents & Guardians:
Our Creswell Middle School Testing Program begins Monday, September 13, 2021 in the library starting at 9:03 am. (Testing will be EVERY Monday at 9:03am in the library.)
In order for your child to participate in testing, you’ll need to register them for the program.

to register your child for the program. Here are a perks to participating in the testing program:
Incentive plan administered by Louisiana Department of Health.
Incentive is opted in on the main electronic consent form.
Incentives are monetary and loaded onto a debit card or online virtual debit account.
The first test incentive is $25. Incentive amount for additional testing is $10 a test. Participants must receive 3 tests in a month to receive payments."
A parent or guardian must register any minor under 18 years old. You’ll give a one-time consent and HIPAA authorization to share their results with the school and district. Testing is not required, but highly encouraged to keep our school community safe.
We’ll test in the Library EVERY Monday starting at 9:03 am. Results will be delivered to you by text or email within 24 hours and we will follow-up with you directly if your child has a positive result.
